Position: Nurse Care Manager – Home Health (RN or LPN)
Location: We serve Southeastern Wisconsin, including Ozaukee, Waukesha, Racine, Kenosha, and Milwaukee suburbs. Preference for candidates located in or around Waukesha and southern Milwaukee suburbs/Racine.
Type: Full-time / Part-time / PRN

Hands-on Medical Support: We provide both short- and long-term care for medical needs such as wound care, catheter care, and medication management.

Aging in Place Assistance: We assess the needs of individuals who wish to stay in their own homes, provide a care plan, and provide the necessary support to make this possible. This may include working with family members or outside caregivers to ensure ongoing safety and well-being.

Key Responsibilities:
Based on physician orders, as well as client and family needs, provide:

Support for Medical Needs: Provide hands-on care, including wound care, catheter care, medication management, and basic assessments.

Aging in Place Assistance: Assess client needs, develop care plans, and support to help them remain safely in their homes, while collaborating with families or external caregivers.

Care Coordination: Help clients with complex health conditions navigate the healthcare system, access resources, and manage their care plans.
Ideal Candidate - Qualifications:

Education & Certifications:
-
Licensed LPN or RN (WI)
-
Additional certifications in CPR, care management, patient advocacy, or social work are a plus.

Clinical Skills:
-
Proficient in basic medical procedures such as taking vitals, wound care, catheter changes. Other advanced skills, like ostomy care, are a plus.

Experience in conducting health assessments and developing care plans, especially during transitions between healthcare settings (hospital, home, facility).

Care Management & Coordination:
-
Ability to navigate the healthcare system, including understanding insurance, financial resources, and available services.

Experience in coordinating care among healthcare providers, family members, and community resources, ensuring seamless patient outcomes.

Ability to manage complex care needs and support clients through medical transitions.

Personal Attributes:

Empathetic and patient, with a strong ability to listen and respond to the needs of clients and their families.

Organized, proactive, and comfortable working independently with minimal supervision.

Strong communication skills, both with clients, families, and interdisciplinary healthcare teams.

Additional Requirements/Preferences:

Travel Requirements: Ability to travel to client homes, healthcare facilities, and appointments as needed. Current/Valid WI Drivers License required; plus reliable/insured vehicle.
